Ülkemizde tipik sosyal fobi türünde olmasa bile topluluğa girme, toplulukta konuşma, toplulukta özgürce davranabilme konularında değişik derecelerde çekingenlik oldukça sık görülen bir durumdur. Bunların büyük bir kısmı klinik düzeyde bir rahatsızlık olarak ele alınmayabilir. Ama gerçekten yüzü kızaracak, elleri titreyecek ya da topluluk içinde yanlış birşey yapacak diye ileri derecede endişe ve kaygıya kapılan ve böyle durumlardan kaçınan kişilerin hekime başvurmaları da oldukça sık görülmektedir. Peki tam olarak sosyal fobi nedir? Bir kişinin diğer insanlar tarafından yargılanabileceği kaygısını taşıdığı toplumsal ortamlarda mahcup ya da rezil olacağı konusunda belirgin ve sürekli korkusunun olduğu bir kaygı bozukluğudur. Kişiler başkalarıyla etkileşimde bulunmalarını gerektiren ya da bir eylemi başkalarının yanında yerine getirmeleri gereken durumlardan korkarlar ve bunlardan mümkün olabildiğince kaçınmaya çalışırlar. Başkalarının kendileriyle ilgili olarak endişeli, özgüvensiz, deli ya da az akıllı olduğu gibi yargılarda bulunacağını düşünürler. Ellerinin ya da seslerinin titrediğinin farkına varacaklarıyla ilgili kaygılarından ötürü toplum önünde konuşmaktan korkabilirler ya da düzgün bir biçimde konuşamıyor gibi görünmekten korktukları için başkalarıyla karşılıklı konuşurken aşırı kaygı duyabilirler. Diğer insanların ellerinin sallandığını görmesinden utanç duyacaklarından korktukları için başkalarının yanında yemekten, içmekten ya da yazı yazmaktan kaçınabilirler. Sosyal fobiyi çekingenlikten ayıran; aşırı kaygı sebebiyle bedensel belirtiler göstermek ve bu bedensel değişikliklerden ötürü de aşırı kaygı duymak, sonuçta da, bu tür durumlardan büyük bir kuvvetle kaçınma isteğinin gözlemlenmesidir. Sosyal fobide korkulan durumla karşılaşıldığında ortaya çıkan bedensel belirtiler; yüz kızarması, terleme, ağız kuruluğu, çarpıntı, nefes kesilmesi, nefes darlığı, mide barsak sisteminde rahatsızlık, diyare, kas gerginliği, titreme olabilir. Bu bedensel tepkilere ek olarak kişinin aklından şu türde düşünceler oluşur: “güçsüzüm, yetersizim, çirkinim, beğenilmiyorum, sevilmeye layık değilim, hata yapmamalıyım, mükemmel olmalıyım, kaygılı olduğumu belli etmemeliyim, rahat davranmalıyım, kusursuz görünmeliyim, herkesin beğenisini kazanmalıyım”. Bu düşünceler sonrasında ise bulununlan ortam ya da durumdan oluşan kaçınma belirtileri; korkulan ortama girmeme, korkulan ortamı terk etme, göz temasından kaçınma, ilgisiz şeyler düşünme şeklinde olabilir. Liebowitz Sosyal Fobi Ölçeğinde belirlenmiş sosyal fobiklerin kaygı duyacağı ve kaçınma davranışı gösterebileceği durumlar şunlardır: • Önceden hazırlanmaksızın bir toplantıda kalkıp konuşmak
• Seyirci önünde hareket, gösteri ya da konuşma yapmak
• Dikkatleri üzerinde toplamak
• Romantik veya cinsel bir ilişki kurmak amacıyla birisiyle tanışmaya çalışmak
• Bir gruba önceden hazırlanmış sözlü bilgi sunmak
• Başkaları içerdeyken bir odaya girmek
• Kendisinden daha yetkili biriyle konuşmak
• Satın aldığı bir malı ödediği parayı geri almak üzere mağazaya iade etmek
• Çok iyi tanımadığı birisine fikir ayrılığı veya hoşnutsuzluğun ifade edilmesi
• Gözlendiği sırada çalışmak
• Çok iyi tanımadığı bir kişiyle yüz yüze konuşmak

DSM-IV’e göre sosyal fobi tanı kriterleri
A. Sosyal ortamlarda ya da performans gerektiren durumlarda veya tanımadık insanlar önünde çıkan belirgin ve inatçı korku. Kişi burada aşağılanmasına veya utanmasına neden olabilecek biçimde davranacağından ya da anksiyete belirtileri göstereceğinden korkar. Not: Çocuklarda, tanıdık kişilerle yaşına uygun toplumsal ilişkilere girebilme becerisi olmalı ve anksiyete yalnızca erişkinlerle olan ilişkilerde değil, akranları ile olan ilişkilerle de ortaya çıkmalıdır.
B. Korkulan toplumsal durumla karşılaşma hemen her zaman anksiyete doğurur. Bu duruma bağlı ya da durumsal olarak yatkınlık gösteren bir panik atak biçimini alabilir. Not: Çocuklarda anksiyete, ağlama, huysuzluk yapma, dona kalma veya tanıdık olmayan insanların olduğu toplumsal durumlardan uzak durma olarak dışa vurulabilir.
C. Kişi, korkusunun aşırı veya anlamsız olduğunu bilir. Not: Çocuklarda bu özellik olmayabilir.
D. Korkulan toplumsal veya performans durumlarında kaçınma, kaygılı beklenti ya da sıkıntının kişinin olağan günlük işlerini, mesleki işlevselliğini (ya da eğitim ile ilgili olan), toplumsal etkinliklerini veya ilişkilerini bozar veya fobi olacağına dair yoğun bir sıkıntı vardır. 18 yaşın altındaki kişilerde süresi en az altı aydır.
E. Korku veya kaçınma bir maddenin (örneğin kötüye kullanılabilen bir ilaç, tedavi amaçlı kullanılabilen bir ilaç) doğrudan fizyolojik etkilerine veya genel tıbbi durumuna bağlı değildir ve başka bir mental hastalıkla daha iyi açıklanamaz (örneğin, agorafobi ile birlikte olan ya da olmayan panik bozukluğu, ayrılma anksiyetesi bozukluğu, vücut dismorfik bozukluğu, yaygın bir gelişimsel bozukluk ya da şizotipal kişilik bozukluğu)
F. Genel bir tıbbi durum veya başka bir mental bozukluk varsa A tanı ölçütünde sözü edilen korku bununla ilişkisizdir. Örneğin kekemelik, parkinson hastalığındaki titreme, veya anoreksia nervosa ya da bulimia nervosadaki anormal yeme davranışına ait korku değildir.
Nedenleri nedir?
Sosyal fobide kalıtsal geçişin rolü çok güçlü olmasa da vardır. Akrabaları arasında sosyal fobik olan kişilerin bu hastalığa yakalanma riski bir miktar daha yüksektir. En önemli etmenlerden biri beyinde birtakım kimyasal ve elektriksel bozukluklar olduğudur. Örneğin, daha önce arkadaşlarının ders anlatırken güldüğü öğrenci, bir sonraki ders anlatımında tahtaya kaygılı ve daha önce verdiği bedensel tepkiler ile çıkabilir. Çocuk yetiştirme biçimi de hastalığın oluşmasında önemli etmendir. Genelde aşırı koruyucu ya da reddedici veya duygusal sıcaklıktan yoksun, katı anne babaların çocuklarında görülebilir. Bazen çocuktan yüksek beklentileri olduğunda bunlara ulaşılamayınca çocuk cezalandırıldıysa başarısızlık korkusu ve buna bağlı olarak yoğun kaygı durumu gelişebilir. Tedavisi Var mıdır? Eğer beraberinde psikiyatrik bir rahatsızlık bulunmuyorsa, erken yaşta fark edildiyse veya kişinin iyileşme isteği yüksekse başarılı tedavi olasılığı da yükselir. Sosyal fobide ilaç tedavisi ve psikoterapi (konuşmaya dayalı ruhsal tedavi) uygulanır. Hastanın durumuna göre bazen tek başına psikoterapi, bazen ilaç tedavisi uygulansa da genelde her ikisinin beraber uygulanmasında başarı daha yüksektir.
