Alkolizm ve Bireyselleşme Sorunları

Alkol, beyin, sinir sistemi,sindirim sistemi, karaciğer, kemik iliği gibi hayati merkezler başta olmak üzere bütün vücudu etkileyen bir maddedir. Etkinin şiddeti alınan alkolün miktarına ve sıklığına bağlı olduğu kadar kişinin duyarlılığına göre de değişir. Alkolün neden olduğu toplumsal,ruhsal,adli vb.kötü sonuçlar bu satırlara sığmayacak kadar fazladır.

Alkolizm,müzmin(kronik),tekrarlayıcı bir hastalıktır.Bu hastalık ilerleyicidir;yani gittikçe kötüleşebilir.Şiddetli alkolizm;şeker hastalığı ya da yüksek tansiyon gibi kronik hastalıklarda olduğu gibi sürekli takibi gerektirir.Aralarda kriz durumu yaşanabilir.

Alkoliklerde alkole karşı fiziksel duyarlılığın yanı sıra bütün istenmeyen sonuçlarına rağmen irade gücü ile yenilemeyen içme ile ilgili zihinsel bir takıntı vardır.Yani bu kişiler alkole bağlı olarak yaşadıkları kötü sonuçlardan ders almazlar.Ve hatta başlarına gelenlerin gerçek nedenlerinin alkol olduğunu inkar ederek teselliyi yine alkolde ararlar. Alkol kullanımı ile ilgili sorunların her geçen gün artması ve bu kullanımla ilişkili bozuklukların tedavisinin de sistematik olarak yapılmasının gerekliliğini doğurmuştur.

Birleşik devletlerde alkol kullanımının toplum sağlığı açısından kalp hastalıkları ve kanserden sonra üçüncü sırada yeralması sorunun önemi açısından iyi bir veri teşkil etmektedir. Ülkemizdeki alkol alış oranı da gözle görülür bir şekilde artış göstermiştir. Ne var ki alkol alış yaşı çok aşağılara inmiştir. Çevremdeki gençlerin alkol alımlarındaki artışla birlikte ortaya çıkan sosyal aktivite bozuklukları,medyanın etkisi,hızlı toplumsal değişme,bireyselleşme sorunları benim bu konuyla ilgili makale yazmama sebep oldu.

Alkol kullanımında bireyselleşme sorunlarını araştırmamın sebebi birçok insanın alkole arkadaş edinme ,sosyalleşme sebebi ile başlaması ve bu unsurun çok fazla göz önüne alınıp irdelenmemesindendir.Genelden özele gidip bu tablonun net bir şekilde görülmesini hedefliyorum.

Alkol Kullanımı ve Alkolizm:

İnsanlık tarihi kadar eski olan alkol kullanımı çağlar boyunca değişik bakış açılarıyla ele alınmıştır. Töresel, bir anlamda sosyakültürel açıdan ;alkol kullanımı kişinin kendi isteği ve iradesiyle medikal anlamda ise;kullanımın hastalıkla ilgili olduğu kabul edilmiştir.Alkol kullanımının sebep ve sonuçlarının kullanan kişinin yanında aile ve toplumu da psikolojik,sosyal,ekonomik açılardan önemli boyutta etkilemesi sorunu “biyopsikososyal” bir model içinde ele alma gereğini doğurmuştur.Olumsuz sonuçlar hem sadece o içme dönemlerinde,hem de uzun süreli içiciliğin getirdiği sorunlar ağlamında kişiyi etkilemektedir.

Trafik kazalarına,şiddete,suç işlemeye,iş kayıplarına ve ölümlere yol açmaktadır.Bu olayların sonuçlarını sadece kişi değil,bütün aile bireyleri ve yakınları çekmektedir. ABD´de alkolü kötüye kullanım ve bağımlılığın bedeli 1983 te 116.9,1988 de 85.9 milyar dolar olarak tahmin edilmiş. Bunun %61i kaybı ve azalan üretkenliğe bağlanırken, sadece %13ü sağlık harcamalarına bağlanmıştır.(Harwood,1985,Rice,1990) Alkol kullanımı ilk ilişkili tanımlamaları tarih boyunca değişmesinin yanında tedavi ,çaba ve programlarına da paralel değişiklikler gözlemlenmiştir. Birleşik devletlerde temeli 1950lerde atılan en yaygın alkol tedavi modeli olarak,Minnesetto modeli kabul edilmektedir.Birleşik devletlerde 1970lerin sonu ve 1980lerin başında tedavi hizmetleri devlet sektöründen yavaş yavaş özel sektör organizasyonlarının eline geçmiştir. Ülkemizde ise 1980lerin başında devlet sektörü yeni yeni devlet sektörü bu organizasyonları kurmaya başlamıştır.İlk “Alkol Tedavi Klinikleri” üniversite bünyelerinde kurulmuş,bunu 1983 yılında Bkırköy Ruh ve Sinir Hastalıkları Hastanesi bünyesinde kurulan Amatem izlemiştir.

Alkolizm deyince birçok insanın zihninde ,parklarda ispirto şişesine sarılıp sızan ağır alkol bağımlıları canlanır.Oysa alkolizm,bireyin beden ve ruh sağlığını,aile,sosyal ve iş uyumunu bozacak derecede sık ve fazla alkol alma isteğini durduramama şeklinde ortaya çıkan bir bozukluktur.

Alkol Kullanma Biçimleri:

Alkol kullanma çok farklı biçimler sergileyebilmektedir.Alkol kullanan her insanda sosyal sorun,sağlık sorunuve buna benzer başka sorunlar oluşmayabilir.Bu kişiler daha çok sosyal içicilerdir.Alkolün bu sorunlarıyla karşılaşmadıkları demek alkolün risklerini kabullenmedikleri anlamına gelmez.Tıbbi sorun yaşayan ,ama alkole bağlı olmayanlar ,alkolü kötüye kullananlar da sosyal içiciler olarak adlandırılır.

Günde 1-2 kadeh içki almanın kalp hastalığı riskini azaltabileceğine işaret eden bilimsel araştırmalar bulunmasına rağmen bazıları için alkol kullanımı kontrol edilemeyen bir saplantıya dönüşür.Alkolü az miktarda problemsiz olarak kullanan pek çok insan olduğu gibi bu nedenle zaman zaman bazı derde giren kişilerve yelpazenin en ucunda alkolsüzyaşayamaz hale gelen ve bu yüzden hayatı felç olmuş insanlar vardır.

Alkol kullanımının tıbbi olarak kabul edilen normal sınırı erkekler için (2), kadınlar için (1) içkidir.Birim olarak bir içki,bir kutu ya da şişe biraya,bir bardak şaraba ya da 45ml´lik bir “tek”sert içkiye(votka,viski vb.) eşittir.Bu şekilde hesaplandığında alınan içkinin türünün hiç bir önemi yoktur.Yani üç bira içmekle üç duble votka içmek aynı miktarda alkol alımına sebep olur ve etkisi aynıdır.

Din ve töreleri ile alkolü onaylamayan toplum kesimlerinde alkolizm oranı daha düşüktür.Alkolün kolay elde edilebirliği ile fazla alkol kullanımı arasında da bir bağ vardır.(Atabek,E;1983)

Alkol kullanımının problem haline dönüşmesi için kişinin sürekli alkol alıyor olması da gerekmez.Kişi zaman zaman kullansa da alkol almaya bağlı olarak aşağıdaki problemlerden birini dahi tekrar tekrar yaşıyorsa profesyonel yardımı gerektirecek düzeyde alkol kullanma problemi var demektir.

1) İşte,okulda ya da evde üstüne düşen görevleri tekrarlayıcı bir şekilde aksatma
2)Fiziksel olarak tehlikeli durumlarda yineleyici biçimde alkol kullanımı
3)Alkol ile ilişkili ortaya çıkan yasal sorunlar
4)Alkolün neden olduğu ya da alevlendirdiği sürekli ya da tekrarlayıcı insanlararası sorunlar(alkolllüyken eşle tartışmalara girmek ya da kavga etmek v.b.)

Toplum alkol kullanımını kontrol edemeyen kişilerin ruhen zayıf hatta dengesiz olduğunu düşünür.Birçok “alkolik” de kendini böyle görür.Ancak alkolizmin bir hastalık olarak görülmesinden asıl kasıt kişinin alkol karşısında iradesini ve seçim gücünü kaybetmesidir.Alkol kullanma karşısında güçsüzlüğünü kabul etme ve bu konuda yardım arama iyiye doğru değişimin ilk adımıdır.Milyonlarca insanın bu ilk adımla başlanan yolda alkolün sosyal hayatlarına verdiği zararın üstesinden gelmiştir.

Alkolizm Tipleri:

a) Gamma Tipi Alkolizm:

Aşırı miktarda alkolün aralıksız biçimde alındığı epizotların yaşandığı,ama aralarda alkol alımının olmadığı dönemlerin olduğu alkolizm tipidir.Örneğin;kişi günler boyunca sızıncaya kadar alkol alır ve ayılı ayılmaz içmeye devam eder.Sağlık durumu nedeniyle içemez hale gelince bir kaç gün hasta yatar.1-2 hafta alkol alamaz,ama sonra her şeye yeniden başlar.Bu kişlerde temel problem alkol aldıkları zaman ortaya çıkan kontrol kaybıdır.Sarhoşluk anında yaptığı şeyleri çoğunlukla hatırlamaz.Yasal ve sosyal problemler ortaya çıkar.Kişi suç işleyebilir,sosyal çevresiyle problemler ortaya çıkar.

b) Fransız Tipi Alkolizm:

Kişi sürekli olarak fazla ama esiri olmayan miktarda alkol alır,alkol kullanımı bir hayat tarzı haline gelmiştir.Herhangi bir nedenle alkol almayı bırakırsa alkol yoksunluğuna girebilir.Uzun vadede sağlık problemleri ortaya çıkar.

c) Tip A-B ya da 1-2:

Bu tip alkolizm çok erken yaşlarda başlar.Genellikle bu kişilerin ailelerinde de alkolizm öyküsü vardır ve alkolizm antisosyal kişilik bozukluğuile birlikte de sıkça görülür.Çok kötü ve sinsi bir ilerleyişi vardır.

d) Daha İyi Gidişli Alkolizm Tipi:

Bu tip alkolizm daha geçyaşta başlar.Aile öyküsü yoktur.Genellikle alkolizme depresyon eşlik eder.Hastanın tedavisi daha kolaydır.(Karamustafaoğlu,O,1981)

Alkol Alımına Bağlı Bozukluklar:

a) Depresyon:

Alkol beyin fonksiyonlarını değiştiren ,bağımlılık yapma potansiyeli yüksek bir maddedir.Alkol etkisi altındaki bir kişinin ruhsal durumunun öngörüsünü yapmak oldukça zordur.

Alkolün bedendeki dağılımı,emilimi,atılım gibi özelliklerinin beyindeki aktivitesinin belirsizliğinden kaynaklanmaktadır.Etkilerinin öngörüsü belirsiz olan bir maddenin kontrolünün sağlanası imkansızdır.Bu belirsizliklerin yanında çok gözlenen ve kabul edilen gerçeklerden biri alkolün ruh ve beden sağlığını olumsuz etkilediğidir. Bugün alkolün ruh sağlığına olumsuz etkilerinden en çok bilineni depresyon dediğimiz klinik durumdur.Depresyon;umutsuzluk,keder,kararsızlık,konsantrasyon bozukluğu ,sıkıntı,suçluluk duyguları,isteksizlik,hız kaybı,uykusuzlukla,bazen de bedensel şikayetlerle seyreden ruhsal bir hastalıktır.

Depresyon toplumda çok sık görülür.Bazen tedavi edilmediğinde intihar davranışı ile sonuçlanan bu hastalığın ,alkol kullanıyla ilgili değişik düzeylerde ilişkisi vardır. Alkol kullanımı depresyona sebep olur.Alkol bağımlıları bağımlı olmayanlara göre depresyona yakalanma açısından oldukça fazla riske sahiptir.Alkol bağımlılarının %80 kadarında depresyon tablosu görülebilir.Bu oran,,genel toplum oranlarının neredeyse dört katıdır.

Alkol bağımlılarındaki depresyon hastalığı genellikle alkolü bıraktıktan sonra fark edilmektedir.Alkol kullanımının devam ettiği dönemlerde ,alkolü kullanan kişi bu belirtileri ( sıkıntı,isteksizlik,huzursuzluk v.s ) alkolü fazla kullanmaya bağlanmakta,çeversindeki kişiler de alkolün gizlediği bu hastalığı fark edemememektedir.Böylece kişide yavaş yavaş bireyselleşme sorunları gözlemlenir.

b) Kişilik Bozuklukları:

1) Alkolü Bırakma Sendromu: Uzun süreli ve düzenli alkol alan kişiler alkolü bırakınca değişik derecelerde bırakma belirtileri gösterirler.Bu belirtiler kısaca şunlardır:Grand-mal epilepsi nöbetleri,terleme,kusma,halsizlik,bulantı,uyku bozukluğu,korkulu düşler kısa süreli görme-işitme ve dokunma hallisünasyonları

2) Delirium Tremens: Kronik alkolizmde alkolü bırakmaya bağlı olarak ortaya çıkan akut beyin sendromudur.Bu bireylerde zamana ve mekana karşı oryantasyon bozulmuştur.

3) Alkol Hallüsünozisi: Uzun süre ve aşırı miktarda alkol alan bireylerde alkol bırakıldıktan sonra ortaya çıkan canlı,sürekli görme ve işitme varsanıları.

4) Korsakof Psikozu(Alkol Amnestik Sendromu): Bireyde yakın ve orta geçmişe ilişkin bellek bozukluğudur.Bireyde yakın ve orta geçmişe ilişkin bellek bozukluğu varken,uzak bellekte bozulma görülmez.

5) Alkol Bunaması (Alkolik Demans): Uzun süreli alkol kullanan bireylerde görülen bunama;burada zaan ve mekana yönelik oryantasyon bozukluğu vardır.

6) Alkol Paranoyası: Kronik alkoliklerde görülen ağır kıskançlık ve aldatılma duyguları ile ortaya çıkan bir psikozdur.

7) Karaciğer Sirozu: Aşırı alkol tüketimi sonucu karaciğer tükenerek işlevini yapamayacak duruma gelmesi ve sonuçta bireyin yaşamını bitirmesidir.(Gençtan,E,1977)

Alkol Alımı Ve Bireyselleşme Sorunları

Alkol alan bir kişide yavaş yavaş birey olmada sorunlar çıkar.Kişi eski özelliklerini kaybeder,kişinin irade gücü ,yaşam tarzı,sosyal ilişkilerinde değişiklikler gözlenir.Fakat kişi bunların değişim gösterdiğini ısrarla reddeder.Ama zaman içinde artan problemler bu gerçeği ortaya çıkarır.Bir çok kişi alkol karşısında aciz olduğunu ve yenik düştüğünü itiraf edene kadar korkunç deneyimler yaşar,maddi manevi büyük kayıplara uğrar,insanlarla ilişkileri bozulur,çalışma hayatları alt üst olur,ruhsal çöküntü yaşarlar.Oysa bu kadar ağır bedel ödemeden ,alkol içmeyi kontrol edemediğini farkedip yardım arayışına girmek en iyisidir.

Alkolikler genellikle “dibe vurmadan” yani her şeylerini kaybetmeden alkolik olduklarını kabul etmezler ya da sırf çevrelerindekileri susturmak için alkolik olduklarını söyler,ama bunu değiştirmek için hiç bir çaba göstermezler.Bu yüzden tedaviye istekli değildirler.

Ailelerin alkolik hastayı destekleyen tavrı “onun dibe vurmasını” ya da dibe vurduğunu farketmesini engeller. Bir başka deyişle;alkolik parasız kalır ,annesi para verir,karakola düşer,babası kurtarır,hasta olur,eşi hastaneye götürür v.s. alklün olumsuz sonuçlarıyla hasta değil,hep ailesi yüz yüze gelir. O ise alkolün verdiği sarhoşluğa sığınıp tüm sorunları inkar eder ya da hep başkalarını suçlar. Bu kadar derdi çeken aile de sürekli olarak alkolü bırakması için alkolik kişiye baskı yapar ve alkolik,bu baskıyı içmesinin nedeni olarak gösterir. “Karımın dırdırından,ailemin baskısından dolayı içiyorum” bahanesini sık sık duyarız. Oysa bu dırdır ve baskı alkole karşıdır.İşte bu durumda bir kısır döngü yaşanmaktadır. Yani alkolün kötü sonuçlarını sırtlanan aile,hastanın bunlarla yüzleşmesini aslında engellerken,yaptıkları baskıyla hastanın stresini daha da arttırırlar.Aileler,özellikle eşler,alkolizmden kendileri sorumluymuşcasına bir suçluluk içinde,kendilerini paralama derecesinde bir kurtarma çabasına girebilirler. Eşler de ,buna bağlı depresyon sıkça görülür.Kişi alkol kullandıkça güvensiz ve bağımlı bir kişilik sergileme boyutuna girerler.Kendini bir bebek gibi yakınlarının bakımına terkeder. Sonuç olarak; alkol kullanımıyla birlikte alkolün olumsuz etkileri kişinin kişilik yapısını etkiler hale geliyorsa acilen müdahale etmek gerekmektedir.

    Die meisten Runden spiele ich mobil, deshalb schaue ich, wie die Lobby auf dem kleinen Bildschirm funktioniert. Der Support zaehlt auch: kommt die Antwort in ein paar Minuten, ist das ein gutes Zeichen.
